Trans-gingival implant placement is a microsurgical rehabilitation protocol that avoids bone resorption due to periosteal detachment when accessed with a full-layer flap. The absence of incisions and sutures excludes the patient's postoperative sensations. This case is interesting in that immediate loading with a temporary crown made by a digital method was performed. If all conditions are met, immediate loading improves osteogenesis around the implant. In combination with the trans-gingival approach, we receive the surgery in one step, improve the quality of the bone around the implant, avoid bone resorption in the area of the implant neck and increase patient loyalty to our practice.
